The Carriage House Campus is a community devoted to Senior Living, encompassing Independent and Assisted living, Short-term rehabilitation and Long-term care. The Carriage House is a 165-bed Medicare/Medicaid-certified health and rehabilitation center.

We provide supervision or assistance with activities of daily living (ADLs) and monitoring of resident activities to help to ensure their health, safety, and well-being.


The Carriage House's Rehabilitation Program is involved in the treatment of disorders of the musculoskeletal system and it’s interaction with physical movement. 


The Carriage House also provides complete skilled nursing and rehabilitation services to meet the post-hospital needs and long-term care needs of our patients and residents.

Alice Cieslinski

Alice was born into the Beaver Township community on March 21, 1916. She was the 9th of 10 children to Francis Janowicz and Ignace Klinski.


She prided herself on early mornings of cow milking, hoeing the garden and evening cow milking before supper. No wonder she be-came such a good cook and baker!

Resident of the Month


Herb Garden

We are starting a herb garden in the main parlor. The garden will be planted and maintained by residents.  Donations of planting pots, starter plants, or seeds would be appreciated.
